Care Services

ALFA Care Services staff help individuals affected by HIV access community resources across our nine-county area: Alexander, Alleghany, Ashe, Burke, Caldwell, Catawba, Lincoln, Watauga, and Wilkes.

Information, Referral, and Supportive Services

ALFA case managers are available Monday–Friday to support people living with HIV in North Carolina’s foothills. We offer confidential help with referrals, forms like HMAP or Medicare, locating HIV-friendly providers, finding grant opportunities, and addressing HIV-related discrimination—often with just a quick call or visit.

Case Management starts with a private Intake Appointment to assess needs across all areas of life. Together, the client and case manager create a Care Plan to work toward personal goals. Over time, many clients reach stability and graduate from case management, but support and referrals remain available anytime.

Often, after some time, clients receiving case management reach their goals, stabilize their lives, and can manage on their own. Their case management file is closed at that point, but they are always welcome to come back for information, referral, and supportive services.
